transsexual

Changed sex medically

A transsexual is a person whose gender identity differs from their sex assigned at birth, particularly one who has undergone or seeks medical transition through hormones or surgery.

The term was standard in medical and psychological literature through the late twentieth century but has largely been replaced by transgender, which many consider more inclusive and less clinical.

Some people still identify as transsexual to emphasize the physical aspect of their transition: She's been a transsexual woman for thirty years.

Both noun and adjective uses occur.
