voting ballot
Marked choice sheet
A voting ballot is the paper, card, or electronic form on which voters mark their choices in an election.
The term is slightly redundant—a ballot is itself a device for voting—but voting ballot emphasizes the act of casting votes and distinguishes the physical object from other senses of ballot: I filled out my voting ballot and dropped it in the box.
