Text in realtime
IM stands for instant messaging, the technology allowing real-time text communication over the internet: We IMed back and forth all afternoon.
As a verb, to IM someone is to send them an instant message.
In medical contexts, IM means intramuscular, describing injections delivered into muscle tissue. (Lowercase im is informal for "I'm.")
