Challenger

Lost 1986 orbiter

Challenger was a NASA space shuttle that exploded seventy-three seconds after launch on January 28, 1986, killing all seven crew members including teacher Christa McAuliffe.

The disaster, caused by a failed O-ring seal, became a defining moment in American spaceflight history and a case study in organizational failure.

The Challenger disaster now serves as shorthand for catastrophic institutional breakdowns. (Lowercase challenger is one who competes or contests.)
