sworn affidavit
Witnessed legal declaration
A sworn affidavit is a written statement confirmed by oath or affirmation, typically before a notary or other official authorized to administer oaths.
The phrase is technically redundant—an affidavit is by definition sworn—but appears frequently in legal and official contexts for emphasis: She submitted a sworn affidavit attesting to the vehicle's condition at the time of sale.
