death screen
Player failure notification
A death screen is the display that appears in a video game when a player's character dies, typically showing options to retry, load a saved game, or return to a menu.
The term also applies to computer error screens that indicate a fatal system crash—most famously the blue screen of death in Windows.
I've seen that death screen so many times I have it memorized.
