Apollo 1
Fatal capsule fire
Apollo 1 was the first crewed mission of NASA's Apollo program, intended to test the spacecraft in Earth orbit.
The mission never flew—a cabin fire during a launch rehearsal in January 1967 killed all three astronauts: Gus Grissom, Ed White, and Roger Chaffee.
The disaster led to major spacecraft redesigns and became a sobering milestone in American space history.
