sick note
Doctor's absence letter
A sick note is a document from a doctor or medical professional confirming that someone is ill and unable to work or attend school.
Employers and schools often require sick notes to excuse absences beyond a day or two: HR said I need a sick note if I'm out more than three days.
Primarily British; Americans more often say doctor's note.
