A fathead is a stupid person—someone whose head seems stuffed with fat rather than brains: Don't be such a fathead.
The insult is mild and somewhat dated, more likely heard in old movies than modern conversation.
Fathead also names several species of fish, including the fathead minnow commonly used as bait.
