butterfly ballot
Staggered punch-hole form
A butterfly ballot is a voting ballot with candidates listed on facing pages, with punch holes running down the center between them.
Voters punch the hole next to their choice, but the staggered layout can cause confusion about which hole corresponds to which candidate.
The term became prominent after the 2000 U.S. presidential election, when Palm Beach County's butterfly ballot led many voters to accidentally select Pat Buchanan instead of Al Gore.
