A juice head is someone who uses anabolic steroids, the "juice" being injectable testosterone or similar compounds.
The term appears in bodybuilding and fitness contexts, often dismissively: Half the guys at that gym are juice heads.
It can also mean a heavy drinker, though this sense is less common today.
