order pad
Waiter's writing booklet
An order pad is a small booklet of blank forms used by waitstaff to write down customers' food and drink requests.
The pages typically have carbon copies so one goes to the kitchen and one stays with the server: She pulled out her order pad and asked what we'd like.
The term extends to similar pads used by salespeople to record purchases.
