exit survey
Leaving feedback form
An exit survey is a questionnaire given to someone as they leave—an employee departing a job, a customer finishing a transaction, a visitor leaving a website.
The goal is to capture feedback while the experience is fresh: The exit survey asked why I was canceling my subscription.
Exit surveys help organizations understand why people leave and what might be improved.
