W-9
Contractor tax form
A W-9 is a U.S. tax form that individuals or businesses complete to provide their taxpayer identification number to someone who will pay them.
Companies typically request W-9s from contractors, freelancers, and vendors before issuing payment: I had to fill out a W-9 before they'd cut my first check.
The information is used to prepare 1099 forms reporting income to the IRS.
