Pink elephants are hallucinated animals supposedly seen by people who are extremely drunk or experiencing delirium tremens from alcohol withdrawal.
The phrase has become a cultural shorthand for drunken hallucinations generally: He's so wasted he's seeing pink elephants.
The image was popularized by the hallucinatory sequence in Disney's *Dumbo*.
