coverage gap
Policy's missing middle
A coverage gap is a situation where insurance or a benefit program fails to cover certain costs, services, or circumstances.
The term is most associated with Medicare prescription drug plans, where beneficiaries pay full price for medications after reaching a spending threshold but before catastrophic coverage begins—informally called the donut hole.
More broadly, any gap in protection: The policy has a coverage gap for flood damage.
