An atomic wedgie is an extreme version of the wedgie—the prank of yanking someone's underwear upward from behind—in which the waistband is pulled up and over the victim's head.
The "atomic" modifier signals escalation, as in other slang compounds: atomic fireball, atomic hot wings.
