skin tear
Superficial tissue damage
A skin tear is a shallow wound caused when fragile skin separates from underlying tissue, typically from friction or shearing force rather than cutting.
Common in elderly patients whose skin has thinned with age: The nurse documented a skin tear on the patient's forearm.
Skin tears often occur during routine handling or when adhesive bandages are removed too quickly.
