douche nozzle
Insufferable rude individual
A douche nozzle is the attachment on a douche used to direct the cleansing stream.
As an insult, it's a more elaborate version of calling someone a douche—implying they're not just unpleasant but specifically the least dignified part of an already undignified apparatus: That douche nozzle cut me off in traffic.
