A pisshead is a habitual heavy drinker—someone who gets drunk frequently or is drunk much of the time.
The term is mildly vulgar British slang, roughly equivalent to American "drunk" or "lush" as nouns: Her ex was a complete pisshead.
It can also be a general insult implying stupidity.
