personal biography
One's own memoir
A personal biography is an account of someone's life, typically written about oneself for professional or promotional purposes.
The phrase is somewhat redundant—all biographies are personal in the sense that they concern a person—but it emphasizes the subject's own involvement or perspective: Please submit a personal biography with your application.
Often shortened to just "bio."
