medical directive
Advance care document
A medical directive is a legal document specifying what medical treatments a person wants or doesn't want if they become unable to communicate their wishes.
Also called an advance directive or living will, it typically addresses life-sustaining measures like ventilators, feeding tubes, and resuscitation: Her medical directive stated she didn't want to be kept alive by machines.
Medical directives may also designate someone to make healthcare decisions on the person's behalf.
