not all there
Missing some marbles
Not all there describes someone who seems mentally deficient, distracted, or slightly crazy.
The phrase suggests something is missing from a person's mental faculties: After the accident, he was never quite all there.
It can be used gently about someone who's spacey or absent-minded, or more pointedly to suggest genuine mental impairment: I love my uncle, but he's not all there.
