Deep pain is physical pain felt in internal structures rather than on the surface—in muscles, joints, bones, or organs rather than skin.
It tends to be dull, aching, and harder to localize than superficial pain: a deep pain in my lower back, deep pain radiating from the chest.
The phrase also describes intense emotional suffering: the deep pain of losing a child.
